ABSTRACT
The tuberculosis infection of the parotid gland is a rare condition and its progress is usually slow. The clinical findings of this entity are nonspesific and differential diagnosis from other parotid masses is challenging. The imaging findings are also nonspesific and the diagnosis is usually made by histo-pathological evaluation. Here we report a 72 years old female patient who applied to our clinic with a parotid mass lesion, which was located on her right neck and progressed within 4 months. In physical examination, a semi-mobile mass about 2 cm in size was detected on right parotid region. The sputum smears negative for acid-fast bacilli are detected. Laboratory findings and plain chest radiography were normal. 2 cm size heterogeneous mass in the parotid gland and adjacent lymphadenopathy were detected by magnetic resonance imaging and ultrasonography. Fine needle aspiration biopsy was done from parotid gland and adjacent lymphadenopathy. Non-caseous granulomas were seen, but no basil was produced. Antituberculosis therapy was given to the patient. The mass lesion showed regression by the treatment so we concluded the diagnosis of parotid gland tuberculosis.

ÖZ
Parotis bezinin tüberkülozu oldukça nadir olup genellikle yavaş seyirlidir. Parotisin diğer kitlelerinden ayırt edilmeleri zor olup, klinik bulgular nonspesifiktir. Görüntüleme bulguları da spesifik olmayıp, tanı genellikle histopatolojik değerlendirme sonrasında konur. Bu makalede, sağ parotis bezi lojunda kitle ile başvuran 72 yaşındaki bayan hastayı sunduk. Bu şişlik 4 ay önce başlamış ve gittikçe büyümüştü. Muayenede sağ parotis bölgesinde yaklaşık 2 cm boyutlarında yarı hareketli kitle saptandı. Balgamda aside dirençli basil negatifti. Laboratuvar bulguları ve akciğer grafisi normaldi. Ultrasonografi ve manyetik rezonans görüntülemede parotis bezinde 2 cm boyutlarında heterojen kitle ve komşuluğunda lenf nodları tespit edildi. Parotis bezinden ve komşu lenfadenopatiden ince iğne aspirasyon biyopsisi yapıldı. Nonkazeifiye granülomlar görüldü, ancak basil üretilemedi. Antitüberküloz tedaviden fayda gören hasta parotis tüberkülozu olarak değerlendirildi.

ABSTRACT
Ewing sarcoma is classified as Ewing’s sarcoma of the bone tissue, extraskeletal Ewing’s sarcoma, peripheral primitive neuroendocrine tumor, malignant small cell tumor of the thoracopulmonary region (Askin) and non-typical Ewing’s sarcoma. Extraosseous Ewing’s sarcoma may be seen throughout the body, however only reported at 4% in the neck. In our case patient has a painless mass growing on his posterior neck for 3 months.On the magnetic resonance imaging, a well-defined, encapsulated, well-demarcated mass lesion between the perivertebral muscles and the subcutaneus fat tissue was detected. Tumor was surgically resected and diagnosis was pathologically Ewing’s sarcoma. 22q11 translocation was detected in the molecular examination. Extraosseous Ewing sarcomas are rarely seen soft tissue masses. Although imaging features are not sufficient to make a specific diagnosis, it is important take the biopsy from the appropriate place and staging the tumor. In addition, complete surgical resection has been shown to associated with better survival rates compared to other Ewing sarcoma family. For this reason, imaging has an important role in the guidance of surgery and resectability of the tumor. Young patients with a fast growing, palpable mass should be evaluated carefully. Although the tumor is thought to be morphologically benign in the first, ekstraosseous Ewing’s sarcoma must be considered in the differential diagnosis of head and neck soft tissue masses.

ÖZ
Ewing sarkomu, kemik dokusunun Ewing sarkomu, iskelet sistemi dışındaki Ewing sarkomu, periferik primitif nöroendokrin tümör, torakopulmoner bölgenin malign küçük hücreli tümörü (Askin) ve tipik olmayan Ewing sarkomu olarak sınıflandırılır. Ekstraosseöz Ewing sarkomu tüm vücutta görülebilir ancak boyunda %4 oranında bildirilmiştir. Olgumuzda, ensesinde 3 aydır giderek büyüyen ağrısız kitlesi olan hastada; manyetrik rezonans görütülemede perivertebral kasların arasında, yağ planlarının arasında büyüyen, kapsüllü, düzgün sınırlı kitle lezyonu saptandı. Cerrahi olarak çıkarılan tümörde patolojik olarak Ewing sarkomu tanısı koyuldu. Moleküler incelemede 22q11 translokasyonu tespit edildi. Ekstraosseöz Ewing sarkomları nadir olarak görülen yumuşak doku kitleleridir. Görüntüleme özellikleri özgün veya tanı koymada yeterli olmasa da, biyopsi için dokunun uygun yerden alınması ve tümörün evrelendirilmesi açısından önemli yer tutar. Ayrıca iskelet dışındaki Ewing sarkomunda, tam cerrahi rezeksiyonun diğer Ewing sarkomu ailesi tümörlerine kıyasla daha iyi sağkalım oranları ile ilişkili olduğu gösterilmiştir. Bu nedenle cerrahiye yön gösterme ve tümörün çıkarılabilirliğinin değerlendirilmesinde görüntüleme önemli yer tutmaktadır. Hızlı büyüyen, ele gelen kitlesi olan genç hastalarda, ilk planda morfolojik olarak benign izlenimi verse de dikkatle değerlendirip baş-boyun yumuşak doku kitlelerinin ayırıcı tanısında düşünülmelidir.

ABSTRACT
Aim: The aim of our study was to evaluate the diagnostic value of three-dimensional contrast-enhanced magnetic resonance angiography (3D CE-MRA) in detecting renal artery stenosis (RAS), and in demonstrating segmental and accessory renal arteries in patients with suspected renovascular hypertension, taking digital subtraction angiography (DSA) as the reference method.
Material and Method: Twenty five patients underwent 3D CE-MRA and DSA. Sensitivity, specificity, positive predictive value, and negative predictive value of CE-MRA in depicting RAS, and sensitivity of the technique in demonstrating segmental and accessory arteries were calculated.
Results: For detecting RAS, the sensitivity, specificity, positive predictive value, and negative predictive value of 3D CE-MRA were; 100%, 97.8%, 87.5%, and 100%, respectively. The sencitivity of the technique in demonstrating segmental and accessory arteries were 58% and 91.7%, respectively.
Conclusion: We found that 3D CE-MRA is a reliable technique in not only detecting RAS, but in demonstrating accessory arteries as well. However, according to our results, the value of the method in visualising segmental arteries is limited.

ÖZ
Amaç: Çalışmamızın amacı, digital subtraction angiography (DSA) tekniğini referans metod alarak, üç boyutlu kontrastlı manyetik rezonans anjiyografi (3D CE-MRA) tekniğinin, renal arter stenozu (RAS) tanısındaki ve segmental ve aksesuar renal arter görüntülemesindeki değerini saptamak idi.
Gereç ve Yöntem: Yirmi beş hastaya 3D CE-MRA ve DSA tetkikleri uygulandı. CE-MRA tekniğinin RAS tanısındaki sensitivite, spesifisite ve pozitif ve negatif kestirim değerleri ile tekniğin segmental ve aksesuar renal arter görüntülemedeki sensitivite değerleri hesaplandı.
Bulgular: RAS tanısında CE-MRA tekniğinin sensitivite, spesifisite, pozitif ve negatif kestirim değerleri, aynı sıra ile; %100, %97,8, %87,5, ve %100 olarak hesaplandı. Tekniğin segmental ve aksesuar renal arter görüntülemedeki sensitivite değerlerinin, aynı sıra ile; %58 ve %91,7 olduğu saptandı.
Sonuç: 3D CE-MRA tekniğinin, sadece renal arter stenozu tanısında değil, aksesuar arterlerin görüntülemesinde de güvenilir olduğu saptandı. Ancak çalışmamızın sonuçlarına göre, tekniğin segmental arter görüntülemesindeki değeri sınırlıdır.

ABSTRACT
Aim: The aim was to investigate the relationship between the location of the humeral head tubercle cysts and the rotator cuff, biceps long head tendon pathologies.
Material and Methods: Magnetic resonance imaging of shoulder in our clinic between February 2014 and March 2018 was retrospectively reviewed. Three hundred patients with the cyst in the humeral head tubercle cysts were included in the study. The greater tubercle was divided into anterior and posterior lines with a parallel line drawn on the humerus shaft in sagittal images. The anterior greater tubercle, the posterior greater tubercle and the lesser tubercle were divided into three groups of cysts. Also according to the size of the cyst, it was divided into 5 mm small and 5 mm large. Pathologies in the rotator cuff tendons were examined as full thickness and partial tears. Pathologies in the long head of the biceps tendon were examined as full-thickness/partial tear and tendonitis-tendinosis.
Results: A total of 123 patients were 57 male (mean age 50.49±11.8) and 66 female (mean age 56.2 ± 11.9). The anterior greater tubercle cysts were found in 50 patients and 41 (82%) of these patients had tears in the supraspinatus tendon. Supraspinatus tendon tear were observed all patients with the anterior greater tubercle cyst larger than 5 mm in cyst size. There were 64 (52%) cysts in the posterior greater tubercle and no significant association with infraspinatus tendon was detected. In lesser tubercle, 22 (18%) patients had cysts, and 10 (45%) of them had tears in subscapularis tendon, 19 (86%) patients observed the long head of the biceps tendon pathology. Thirteen (10.5%) patients had cysts in different tubercles.
Conclusion: In the study, the posterior greater tubercle cysts are the most common cysts, and no significant a relationship was found with age and tendon pathology. The anterior greater tubercle cysts were observed widely torn in the supraspinatus tendon. The lesser tubercle cysts are seen less frequently and are associated with subscapularis tendon and the long head of the biceps tendon pathologies.

ÖZ
Amaç: Humerus başı tüberkül kistlerin lokalizasyonu ile rotator manşet ve biseps uzun başı tendonu patolojileri arasındaki ilişkinin araştırılması amaçlandı.
Gereç ve Yöntemler: Şubat 2014-Mart 2018 tarihleri arasında kliniğimizde çekilen omuz manyetik rezonans görüntüleri retrospektif olarak incelendi. Humerus başı tuberkül kisti olan 123 hasta çalışmaya dahil edildi. Büyük tüberkül, sagital görüntülerde humerus şaftına çizilen paralel çizgi ile ön ve arka olarak ayrıldı. Ön büyük tüberkül, arka büyük tüberkül ve küçük tüberkül olarak kistler üç gruba ayrıldı. Ayrıca kist boyutlarına göre 5 mm küçük ve 5 mm büyük olmak üzere ayrıldı. Rotator manşet tendonlarındaki patolojiler tam kat ve parsiyel yırtık olarak incelendi. Biseps uzun başı tendonundaki patolojiler tam kat/parsiyel yırtık ve tendinit-tendinozis olarak incelendi.
Bulgular: Toplam 123 hastanın 57’si erkek (yaş ortalaması 50,49±11,8), 66’sı kadındı (yaş ortalaması 56,2±11,9). Ön büyük tüberkülde 50 (%40) hastada kist tespit edildi ve bu hastaların 41 (%82)’inde supraspinatus tendonunda yırtık mevcuttu. Ön büyük tüberküldeki kist boyutu 5 mm’den büyük olanların hepsinde supraspinatus tendonunda yırtık izlendi. Arka büyük tüberkülde 64 (%52) hastada kist mevcut olup infraspinatus tendonu ile anlamlı bir birliktelik saptanmadı. Küçük tüberkülde 22 (%18) hastada kist mevcuttu, bunlardan 10 (%45) hastada subskapularis tendonunda yırtık, 19 (%86) hastada biseps uzun başı tendonu patolojisi izlendi. 13 (%10,5) hastada farklı tuberküllerde kistler mevcuttu.
Sonuç: Çalışmamızda, arka büyük tüberkül kistleri en sık görülen kistler olup yaş ve tendon patolojisi ile anlamlı bir ilişki bulunmadı. Ön büyük tüberkül kistleri olanlarda supraspinatus tendonunda yaygın olarak yırtık izlendi. Küçük tüberkül kistleri daha az sıklıkta görülmekte olup subskapularis tendon ve biseps uzun başı tendonu patolojileri ile birliktelik göstermektedir.

ABSTRACT
Cat Scratch Disease; is a slow-onset infectious disease characterized by regional lymphadenopathy, mostly seen in children and young adults. The disease affects Bartonella henseleae. The agent is often transmitted to humans by the cat. The disease is presented with regional lymphadenopathy, fever, fatigue and weakness. 25-year-old female patient with a right inguinal region swelling and abdominal pain acomputerized tomography (CT) scan ;In the right inguinal region was observed 5x4 cm sized centralized necrotic LAP. Abdominal Magnetic Resonance Imaging (MRI); liver and spleen multipled by 3cm in diameter showed hyperintense in T2 Weighted Imaging (WI), peripheral enhancement in contrast images and lesions compatible with abscess showing diffuse restriction in diffuse MR Histopathological examination of inguinal lymph node excisional biopsy; "Granulomatous lymphadenitis compatible with Cat Scratch". In patients with unexplained fever, LAP and visceral organs should be kept in mind when abscess is present.

ÖZ
Kedi Tırmığı Hastalığı; daha çok çocuklarda ve genç erişkinlerde görülen, bölgesel lenfadenopati ile karakterize yavaş seyirli bir enfeksiyon hastalığıdır. Hastalığın etkeni Bartonella henseleae’dır. Etken, sıklıkla kediler tarafından insanlara bulaştırılır. Hastalık; bölgesel lenfadenopati, ateş, halsizlik ve yorgunluk gibi yakınmalarla kendini gösterir. Sağ inguinal bölgede şişlik ve karın ağrısı şikayetiyle başvuran 25 yaşında bayan hastanın bilgisayarlı tomografi (BT) incelemesinde; sağ inguinal bölgede 5x4 cm boyutunda santrali nekrotik LAP izlendi. Abdominal Manyetik Rezonans Görüntüleme (MRG)'de; karaciğer ve dalakta multipl büyüğü 3 cm çapında T2 Ağırlıklı Görüntüleme (AG)'de hiperintens kontrastlı imajlarda periferal kontrastlanan difüzyon MR'de difüzyon kısıtlanması gösteren apse ile uyumlu lezyonlar izlendi. İnguinal lenf bezi eksizyonel biyopsisinin histopatolojik incelemesi; “Kedi Tırmığı Hastalığı ile uyumlu granülomatöz lenfadenit” olarak tanımlandı. Nedeni bilinmeyen ateşle gelen hastalarda LAP ve visseral organlarda apse birlikteliğinde KTH akılda bulundurulmalıdır.

ABSTRACT
Aim: The aim of our study is to evaluate the clinical and radiological features of children with neurofibromatosis type 1.
Material and Method: The patients who were followed up and treated with neurofibromatosis type 1 in Department of Pediatric Neurology at Sutcu Imam University between the years of 2013-2016 were retrospectively evaluated clinically and radiologically.
Results: Twenty one children were included in this study. Of those, 11 were boys and 10 were girls. Fourteen cases (28.6%) had a family history. The mean age of the children was 10.41±3.05 years. Lisch nodules were observed in 11 patients, and axillary freckling in 7 patients. There were neurofibromas in 5 cases (23.8%). Plexiform neurofibromas were not detected in any of the cases. Scoliosis was seen in 4 patients (19%). Six of the cases (28.5%) had learning difficulties.
Conclusion: Early diagnosis of NF-1 is very important to inform families about the disease and prevent treatable complications with regular follow-up of these children.

Amaç: Çocukluk çağı nörofibromatozis tip 1 olgularımızın klinik ve radyolojik bulgularının değerlendirilmesi
Gereç ve Yöntem: 2013-2016 yılları arasında Sütçü İmam Üniversitesi Tıp Fakültesi Çocuk Nöroloji Bilim Dalı’nda nörofibromatozis tanısı ile takip ve tedavileri yapılan hastaların klinik ve radyolojij özellikleri retrospektif olarak değerlendirildi.
Bulgular: Çalışma grubuna 21 çocuk dahil edildi. Bunlardan 11’i erkek ve 10’u kızdı. Yaş ortalaması 10.41±3.05 yıl saptandı. On dört (%28,6) olguda aile öyküsü mevcuttu. On bir (%52,3) hastada Lisch nodülü ve 7 (%33,3) hastada koltukaltı çillenmesi görüldü. Olguların 5 (%23,8)’inde nörofibrom vardı. Olguların hiçbirinde pleksiform nörofibrom saptanmadı. Dört (%19) hastada skolyoz görülmüştür. Olguların 6’sında (%28,5) öğrenme güçlüğü vardı.
Sonuç: Nörofibromatozis tip 1’in erken tanısı; ailelerin hastalık hakkında bilgilendirilmesi ve bu çocukların düzenli klinik takibi ile tedavi edilebilir komplikasyonları önlemek açısından çok önemlidir.

ABSTRACT
Herpes simplex virus-1 (HSV-1) is a double-stranded DNA virus and is the most common cause of fatal sporadic encephalitis world-wide. HSV-1 encephalitis is characterized by rapid onset of fever, headache, altered mental status, seizures and focal neurological symptoms. Paraneoplastic and autoimmune limbic encephalitis is a condition which presents with similar signs and symptoms, and which develops as a result of antibodies raised against the tumor or synaptic proteins. In patients with this condition, bacterial and viral causes should be excluded, particularly HSV-1 encephalitis. In this report, we present a rare case of limbic encephalitis associated with HSV-1 virus.

ÖZ
Dünyadaki ölümcül sporadik ensefalitlerin en sık etkeni olan herpes simplex virüs-1 (HSV-1); çift sarmallı DNA virüsüdür. HSV-1 ensefaliti klinik olarak hızlı başlangıçlı ateş, baş ağrısı, bilinç değişiklikleri, nöbetler ve fokal nörolojik belirtiler ile karakterizedir. Benzer semptom ve bulgularla seyreden paraneoplastik ve otoimmün limbik ensefalit ise, tümör veya sinaptik proteinlere karşı oluşan antikorlarla gelişmekte ve bu hastalarda bakteriyel ve viral nedenlerin, özellikle HSV-1’e bağlı gelişen ensefalitin ekarte edilmesi gerekmektedir. Bu makalede HSV-1 virüsü ile ilişkili limbik ensefalit gelişen bir olgu sunuldu.

ABSTRACT
A 15 year old patient presented with a two year old mass on her right malar area that started growing in the last few months. On physical exam, subcutaneous, solid, non-tender, mobile mass was palpated. On ultrasonography; in the subcutaneous fat tissue of the right malar area a hypoechoic solid mass with lobulated contours and microcalcifications, was seen. On magnetic resonance imaging; identified the characteristics of the mass enhancement. The group performed with magnetic resonance imaging enhancement features were identified.Excisional biopsy was performed and diagnosed as solitary fibrous tumor.

ÖZ
Sağ yanağında iki yıldır ağrısız kitlesi olan 15 yaşında kadın hasta, kitle boyutlarında son aylarda artış şikâyeti ile başvurdu. Fizik muayenede sağ yanakta, cilt altında sert, hareketli hassas olmayan kitle palpe edildi. Olgunun yüzünde hissizlik veya fasyal paralizi yoktu. Ultrasonografi incelemesinde; sağ yanakta cilt altı yağ dokuda, içerisinde mikrokalsifikasyonların izlendiği, lobüle konturlu, hipoekoik solid kitle lezyonu saptandı. Yapılan Manyetik rezönans görüntüleme ile kitlenin kontrastlanma özellikleri tanımlandı. Olguya eksizyonel biyopsi yapılarak soliter fibröz tümör tanısı patolojik olarak doğrulandı.
